Health care on the go with new MyChart mobile app
MyChart, the free online health record for Providence patients, is now available as a mobile application for iPhone and Android smartphones. Patients can use the MyChart app to view test results, send messages to their care team, schedule appointments, request prescription renewals and more. So whether you're at home, at work or on the road, your health information is just a tap away.
MyChart is available at most Providence Medical Group clinics in Oregon and southwest Washington and a limited version is available at participating Providence hospitals.

If you're already a MyChart user, it's easy to get started. Simply download the app from the iPhone app store or the Google Play app store - search for "MyChart" from Epic. Then open the app, select "Providence-California, Oregon, SW Washington" as your health care provider and login with your Providence MyChart ID and password.

Signing up is easy. You can sign up online if you: are 18 years of age or older, have an established relationship with a Providence primary care provider or have been a patient at a participating hospital in the last 12 months, and have a social security number on file.
Participating locations include: Providence clinics in Oregon and southwest Washington (except in the Hood River and Seaside areas), Providence Portland Medical Center, Providence St. Vincent Medical Center, Providence Willamette Falls Medical Center and Providence Medford Medical Center. MyChart will be available at all Providence locations by August 2013.
