Providence ElderPlace is an innovative care model known as PACE (Program of All Inclusive Care for the Elderly), a federally recognized, comprehensive care program that offers health, housing, social service and care coordination solutions to older adults. Providence ElderPlace in Laurelhurst is one of eight Health and Social Centers where members receive integrated PACE services.
The ElderPlace in Laurelhurst site is located in the Laurelhurst neighborhood of Portland, at 4540 NE Glisan St., kitty-corner from Providence Portland Medical Center. ElderPlace participants receive medical and social services while attending the Laurelhurst Health and Social Center. (read more)
