Eligible individuals are age 55 or older; living in or willing to relocate to Multnomah County; in need of support services as defined by the state of Oregon; and able to live in a community setting.
Eligible individuals are:

- able to live in a community setting such as their own home, an assisted living facility or an adult care home.
Providence ElderPlace eligibility guidelines are mandated as a federally recognized, nationwide PACE program.
