Providence ElderPlace in Gresham is one of eight Health and Social Centers that make up Providence ElderPlace. Providence ElderPlace is an innovative program known as PACE (Program of All Inclusive Care for the Elderly), a federally recognized, comprehensive care program that offers health, housing, social service and care coordination solutions to older adults and peace of mind to their families.
The ElderPlace in Gresham site is located in east Portland, at 17727 E. Burnside Street. ElderPlace participants receive medical and social services while attending the Gresham Health and Social Center. (read more)
