Our services are available to patients and families in Washington, Multnomah, Clackamas, Yamhill, Jackson, Josephine, Hood River, Wasco, Sherman, Gilliam, Morrow, Wheeler and parts of Columbia and Marion counties. In Washington state we serve Klickitat and Skamania counties.
We serve patients in their home, whether it is a private residence or located in an assisted living community, residential care facility, adult care home or nursing facility.
