Transitional Care
"Transitional care" encompasses the services provided to residents, who require short-term, specialized assistance. It is designed to help the resident make the transition home or to an institution or other setting which can provide a lower skill-level of appropriate care The length of stay is based on the progress of each individual resident, and ranges from a few days, to three months or more for intensive rehabilitation and therapy. Discharge arrangements are made by our social services director in consultation with the patient's personal physician.
Long-Term Care
"Long-term care" encompasses skilled services provided to those who need assistance with daily living activities. All of our residents receive 24-hour nursing care to dispense the medications and treatments ordered by the physician's personal physician. Each care plan reflects the individuals special needs and abilities. Families are encouraged to participate in the plan of care, as an important part of the care team.
