Providence ElderPlace Health & Social Centers

Providence ElderPlace, a PACE (Program of All-Inclusive Care for the Elderly) model of care, helps keep older adults as healthy as possible in the community by providing comprehensive health care and social services including: primary and specialty medical care, a day health program, nursing, therapy and social work services, rehabilitation, housing (if necessary) and much more.

Because Providence ElderPlace assists with scheduling, transportation and other arrangements, our members can be certain they will receive the care they need when they need it. We coordinate the delivery of care so our members and their families are free to enjoy their time together.
