Eligible individuals are:
- Age 55 or older;
- Living in or willing to relocate to Multnomah County or designated areas of Washington County;
- In need of support services as defined by the state of Oregon; and
- Able to live in a community setting
Providence ElderPlace eligibility guidelines are mandated in its role as a federally recognized, nationwide PACE program.
